Charge for Primary Procedure: $1650 per eye
Charges for additional treatment: After 12 months, $825 per eye
Charges include:
- Preoperative TLC screening session
- Preoperative discussion session with Dr. Jaben
- Surgical procedure at TLC
- Postoperative care and visits for 12 months
- Any enhancement for 12 months (except for a $250 equipment charge at TLC)
*** Fees do not include any charges for the initial complete general eye exam at Charlotte Eye Ear Nose and Throat Associates with Dr. Jaben. This may be covered by customary health plans depending on the plan and the coverages within that plan.
Charges for this exam MAY be waved by Dr. Jaben if a recent eye exam has been performed by a Charlotte Eye Ear Nose and Throat Associates eye doctor. The exam must contain the appropriate data needed to understand the patient's vision problem and the records must be reviewed by Dr. Jaben.
